Paliativní péče v domovech pro seniory: 5 let zkušeností z programu nadačního fondu Abakus

The final conference will present the results and examples of good practice of two years of the ABAKUS palliative care program in residential social services for the elderly. We welcome the participation of employees and co -workers of residential social services for the elderly, representatives of professional and roof companies, regional officials and representatives of the founders. Nevertheless, their inhabitants in 40 % of cases die in or from transporting them, ie outside their familiar environment. Thanks to the support of the Abakus Endowment Fund, the Palliative Care Center and Sue Ryder have been working on a program since 2021, which has been introducing palliative care to residential facilities for the elderly. We will present the results of the program together at this conference.

The aim of the conference is to help the scaling of the implementation program and the dissemination of good practice from supported organizations, as well as to support regional and supra -regional cooperation.
